Next Level Health AZ provides telehealth consultations and ongoing care primarily via secure video and phone appointments. By scheduling a telehealth visit, you acknowledge that telehealth services have inherent limitations compared to in-person care and that certain conditions may require an in-person evaluation.

Telehealth services are subject to applicable Arizona state law and federal regulations. You must be located in Arizona at the time of your telehealth appointment. We reserve the right to decline or discontinue telehealth services at our discretion.

Cash-based practice and fees

Next Level Health AZ is a cash-based practice and does not bill insurance. All fees are due at the time of service. By scheduling an appointment, you agree to pay the applicable fees as disclosed at the time of booking.

We are not responsible for any out-of-pocket costs, denied claims, or coverage decisions made by your insurance provider. We recommend contacting your insurer directly if you have questions about potential reimbursement.

We do not share mobile phone numbers, text messaging opt-in data, or SMS consent with any third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the text message services used to operate this program.
